目的探讨全身麻醉诱导期正压通气至肌松起效时,肺顺应性及其变化率与肌松药起效时间的相关性。方法选择全麻下择期妇科腹腔镜手术患者35例,年龄18~50岁,体重45~80 kg,ASAⅠ或Ⅱ级,Mallampati 1—2级。麻醉诱导自主呼吸消失后行面罩压力控制通气,通气压力为15 cmH_2O,每12秒记录肺顺应性,以第一次正压通气的肺顺应性为基础值。同时给予顺式阿曲库铵,肌松监测仪定标开始监测,间隔12 s直至T4/T1=0即肌松药起效,记录肌松起效时间和此时肺顺应性数值,计算肺顺应性变化率。结果给予肌松药即刻肺顺应性为(23.1±2.5)ml/cmH_2O,当T4/T1=0时,肺顺应性为(44.6±8.7)ml/cmH_2O。平均肌松起效时间为(87.1±7.3)s,肺顺应性变化率与肌松药起效时间呈负相关(r=-0.68,P=0.013)。结论全麻诱导时肺顺应性变化率与肌松药起效时间负相关,观察肺顺应性的变化有助于更准确地判断气管插管时机。 相似文献

目的 探究唤醒状态联合中央沟定位对脑运动区胶质瘤切除术患者运动功能的保护作用。方法 选取本院2016年12月—2017年10月期间收治的脑运动区胶质瘤患者70例。所有患者均行唤醒状态联合中央沟定位运动区胶质瘤切除术。观察患者术后复发情况及术后第6、12个月的Kamofsky评分,并记录并发症发生情况。结果 所有患者均成功完成手术,未出现术中大出血、死亡等严重并发症。70例患者中,有40例全切除,26例次全切除,4例大部分切除。术后,感觉减退者有6例,肌力下降者有5例,肌力下降伴感觉减退者有2例,并发症发生率为18.57%(13/70)。6个月后,应用增强MRI显示有4例患者肿瘤灶周围明显强化,进一步使用正电子发射断层显像发现有1例患者肿瘤复发。术前及术后第6、12个月的Kamofsky评分分别为(89.62±11.88)及(80.73±9.91)、(65.82±12.36)。结论 唤醒状态联合中央沟定位能最大限度切除脑运动区胶质瘤,提高患者预后效果。 相似文献

BackgroundWe present the care of 17 consecutive pregnant patients who required mechanical ventilation for Coronavirus Disease 2019 (COVID-19) pneumonia at a quaternary referral center in the United States. We retrospectively describe the management of these patients, maternal and fetal outcomes, as well as the feasibility of prone positioning and delivery.MethodsBetween March 2020 and June 2021, all pregnant and postpartum patients who were mechanically ventilated for COVID-19 pneumonia were identified. Details of their management including prone positioning, maternal and neonatal outcomes, and complications were noted.ResultsSeventeen pregnant patients required mechanical ventilation for COVID-19. Thirteen patients received prone positioning, with a total of 49 prone sessions. One patient required extracorporeal membrane oxygenation. All patients in this series survived until at least discharge. Nine patients delivered while mechanically ventilated, and all neonates survived, subsequently testing negative for SARS-CoV-2. There was one spontaneous abortion. Four emergent cesarean deliveries were prompted by refractory maternal hypoxemia or non-reassuring fetal heart rate after maternal intubation.ConclusionsOverall, maternal and neonatal survival were favorable even in the setting of severe COVID-19 pneumonia requiring mechanical ventilation. PurposeTo associate medications, anesthetic techniques, and clinical conditions that interfere in the time of patient approval in the safety protocol for thirst management.DesignA quantitative, analytical, and longitudinal study conducted in Southern Brazil.MethodsA nonprobabilistic sample, of 203 adult patients in the immediate postoperative period, evaluated every 15 minutes for 1 hour.FindingsA general prevalence of thirst of 67.7%, and mean intensity of 6.38. Fentanyl, morphine, rocuronium, and sevoflurane increased lack of approval in the protocol within 30 minutes (P < .05). General anesthesia (P < .0001) and level of consciousness (95.4%) presented the highest nonapproval rates.ConclusionsAnesthetics and general anesthesia delayed protocol approval; however, after 30 minutes, 75.4% of patients had been approved. Level of consciousness was the main criterion of disapproval. The protocol identified crucial clinical conditions that made it impossible for the patient to receive thirst relief strategies and demonstrated that thirst can be satiated precociously with safety. 相似文献
